Align Technology shipped a record 691,785 Invisalign clear aligner cases in the second quarter of 2026, up 7.4% year over year, and more than 23.5 million patients have now been treated with its system (Align Technology, Q2 2026 earnings release). Adults are the core of that business: the American Association of Orthodontists says one out of three orthodontic patients is an adult, and Align’s 2025 numbers imply that roughly 64% of its cases went to adults. Yet the category is still small next to the need, with only about 22 million people a year starting orthodontic treatment out of an estimated 600 million who could benefit. The direct-to-consumer wave that promised to close that gap collapsed, with SmileDirectClub filing for Chapter 11 in 2023 and Byte stopping new patients in 2025. We aggregated data from Align Technology’s SEC filings, the American Association of Orthodontists, SmileDirectClub and Dentsply Sirona filings, the Straumann Group, the ADA Health Policy Institute, peer-reviewed systematic reviews, and market research firms.
TL;DR
- Align shipped 2,611,280 clear aligner cases in 2025, up 4.7% from 2,493,735 in 2024 (Align Technology, FY2025 earnings release).
- Q2 2026 volume hit a record 691,785 cases, up 7.4% year over year (Align Technology, Q2 2026 earnings release).
- One out of three orthodontic patients are adults (American Association of Orthodontists, Press Room).
- 935.8 thousand teens and kids started Invisalign treatment in 2025, implying about 64% of Align volume was adult (Align Technology, FY2025 earnings release).
- Adult patient volume grew 8% year over year in Q4 2025 vs 6.9% for teens and kids (Align Technology, Q4 2025 earnings release).
- Only about 22 million people a year start orthodontic treatment, out of about 600 million who could benefit (Align Technology, Form 10-K 2025).
- Align estimates its share of those 22 million annual case starts at about 10% (Align Technology, Form 10-K 2025).
- SmileDirectClub filed for Chapter 11 on September 29, 2023 while serving over 2 million customers (SmileDirectClub, 8-K October 2023).
- Dentsply Sirona stopped offering Byte aligners to new patients in January 2025 (Dentsply Sirona, Form 10-K 2024).
- Refinements were needed in over 50% of aligner cases and 17.2% were converted to fixed appliances (Borzabadi-Farahani, Frontiers in Dental Medicine 2026).
- Aligner patients reported less pain than fixed-appliance patients (SMD -0.419) across 32 studies (BDJ Open meta-analysis 2026).
- Market forecasts range from 11.12 billion dollars in 2026 (Precedence Research) to 11.04 billion dollars by 2031 (MarketsandMarkets).
1. Case Volume and Market Scale: A Big Business in a Mostly Untreated Market
Clear aligners are a mature category by revenue and an early one by penetration. Align Technology shipped 2,611,280 clear aligner cases in 2025 and generated about 3.2 billion dollars of clear aligner revenue, roughly 80% of its 4.0 billion dollars in total revenue (Align Technology, Q4 and FY2025 earnings release, SEC 8-K). Growth has reaccelerated from a flat 2024: 2025 volume rose 4.7%, Q4 2025 rose 7.7%, and Q2 2026 rose 7.4% to a record 691,785 cases (Align Technology, Q2 2026 earnings release).
The real story is the gap between need and treatment. Align’s 2025 Form 10-K estimates that 60% to 75% of the global population has malocclusion and about 600 million people could benefit from straightening their teeth, yet only about 22 million people a year elect treatment with orthodontists. Align puts its own share of those case starts at about 10%, and most cases are still treated with wires and brackets. Volume is no longer the question for the category leader; conversion of untreated adults is.

| Clear aligner revenue, 2025 | About 3.2 billion dollars (+0.5% YoY) | Align Technology, FY2025 earnings release |
| Clear aligner revenue, Q2 2026 | 870.9 million dollars (+8.2% YoY) | Align Technology, Q2 2026 earnings release |
| Cumulative Invisalign patients | 23.5 million (Q2 2026) | Align Technology, Q2 2026 earnings release |
| People with malocclusion who could benefit | About 600 million (60% to 75% of global population) | Align Technology, Form 10-K 2025 |
| Annual orthodontic case starts worldwide | About 22 million; Align share about 10% | Align Technology, Form 10-K 2025 |
Context note: the AAO’s own estimate is that 50-75% of people could benefit from orthodontic treatment (AAO Press Room), close to Align’s 60% to 75% malocclusion range. SmileDirectClub’s 2022 10-K described a similar opportunity of about 500 million potential customers and about 20 million annual case starts.
2. Adults vs Teens: Adults Are the Majority of Aligner Volume
Orthodontics is still widely seen as a teenage rite of passage, but the patient base says otherwise. One out of three orthodontic patients are adults, according to the American Association of Orthodontists, alongside an estimated 3.9 million children in orthodontic treatment in the US (AAO Press Room). In clear aligners the adult tilt is far stronger. Align reported a record 935.8 thousand teens and kids starting Invisalign treatment in 2025; set against 2,611,280 cases shipped, that implies about 1,675,480 adult cases, or about 64% of volume (2,611,280 minus 935,800, divided by 2,611,280). The two counts are not perfectly comparable (treatment starts vs case shipments), so treat 64% as an approximation.
Adults are also not the slow-growing segment. In Q4 2025, adult patient volume rose 8% year over year versus 6.9% for teens and kids, while full-year teen and kid starts grew 7.8%. Of the more than 22 million cumulative Invisalign patients at the end of 2025, over 6.5 million were teens and growing kids, which leaves roughly 15.5 million adults and older patients (22 million minus 6.5 million). Demand reaches into old age: a 2026 Korean study of 146 orthodontic patients aged 60 and older found 48.6% came in for anterior crowding, and 70.9% of those diagnosed accepted active treatment (Lee et al., Korean Journal of Orthodontics 2026).
| Metric | Value | Source |
|---|---|---|
| Adult share of orthodontic patients | One out of three | AAO, Press Room |
| Children in orthodontic treatment, US | An estimated 3.9 million | AAO, Press Room |
| Teens and kids starting Invisalign, 2025 | 935.8 thousand (+7.8% YoY, record) | Align Technology, FY2025 earnings release |
| Implied adult share of Align volume, 2025 | About 64% (derived, see math above) | Align Technology, FY2025 earnings release |
| Adult patient volume growth, Q4 2025 | +8% YoY (teens and kids +6.9%) | Align Technology, Q4 2025 earnings release |
| Cumulative teen and kid Invisalign patients | Over 6.5 million of 22 million+ | Align Technology, FY2025 earnings release |
| Patients 60+ citing anterior crowding (Korea) | 48.6% of 146; 70.9% accepted treatment | Lee et al., Korean Journal of Orthodontics 2026 |
| Patients 60+ referred from other specialties | 75.3% (24.7% sought care directly) | Lee et al., Korean Journal of Orthodontics 2026 |
Context note: the AAO adult share covers all orthodontic patients (braces and aligners), while the Align split is for one brand. In the Korean senior cohort, 94.9% retained at least 20 natural teeth, a reminder that better tooth retention is what makes late-life orthodontics possible.
3. Doctors, Channels and DSOs: Where Adult Aligner Cases Are Delivered
The adult aligner market runs through two very different channels. About 130,015 Invisalign-trained doctors were active at the end of 2025, defined as having submitted at least one case in the prior 12 months (Align Technology, Form 10-K 2025), against the AAO’s 19,000 orthodontist members across the US, Canada and abroad (AAO, About). That math means most active aligner prescribers are general dentists, not orthodontists. Yet growth skewed to specialists in Q4 2025: volume rose 8.9% in the orthodontic channel and 5.3% in the GP dentist channel.
Utilization is creeping up: 87,710 doctors received shipments in Q4 2025 at 7.7 cases each (85,685 at 7.3 a year earlier), and 89,175 doctors averaged 7.8 cases in Q2 2026, which Align attributes partly to “continued double-digit growth from DSOs” (Align Technology, Q2 2026 earnings release). Competitors are chasing the GP channel too: the Straumann Group says ClearCorrect “continued to expand its presence among general practitioners” and is on course to break even by the end of 2027 (Straumann Group, H1 2026 results release). Software matters in this channel, with case assessment and outcome simulation now sold as GP features, a theme covered in our AI in healthcare statistics.

| Orthodontists starting 50+ aligner cases a year, Turkey vs Iran | 33.1% vs 7.7% | Borzabadi-Farahani et al., Frontiers in Dental Medicine 2026 |
Context note: Align’s Q4 2025 release also cites “over 295 thousand active Invisalign trained doctors” in its milestones section, which does not match the 10-K’s 130,015 active doctors; we use the 10-K figure because it states its definition. The Turkey and Iran survey (Frontiers in Dental Medicine 2026) had a 7.5% response rate in Turkey (178 orthodontists), so self-selection is likely; 86.4% of Turkish respondents used Invisalign and 21.5% ClearCorrect, while 49.3% of Iranian respondents used local manufacturers.
4. The Direct-to-Consumer Collapse: SmileDirectClub and Byte
The mail-order model was built for adults who wanted straighter teeth without an orthodontist’s office, and it did not survive. SmileDirectClub filed for Chapter 11 on September 29, 2023 in the US Bankruptcy Court for the Southern District of Texas while serving over 2 million customers (SmileDirectClub, 8-K Exhibit 99.1, October 2023). The warning signs were in its last full year: 2022 revenue fell 26.2% to 471 million dollars, the net loss was 278 million dollars, and Q4 2022 shipments dropped to 41,462 (SmileDirectClub, Q4 2022 earnings release).
The model was cheap, not free. SDC’s 2022 Form 10-K priced treatment generally at 2,050 dollars, up to 60% less than traditional options, and about 60% of customers financed it through SmilePay at an average of 89 dollars a month. Byte followed: Dentsply Sirona reduced Byte operations after October 24, 2024, after voluntarily suspending sales and marketing, then announced in January 2025 it would no longer offer Byte to new patients (Dentsply Sirona, Form 10-K 2024). The adult demand these companies tapped is now served almost entirely through dental offices, where Align’s orthodontic and GP channels both grew in Q4 2025. The broader question of how much care can safely happen remotely is tracked in our telehealth statistics.

| Byte status | Ops reduced after Oct 24, 2024; no new patients from Jan 2025 | Dentsply Sirona, Form 10-K 2024 |
Context note: the AAO’s October 29, 2024 guidance to Byte patients advised them to see a licensed orthodontist in person and avoid other mail-order companies that skip X-rays and in-person exams (AAO, Guidance to Byte Patients). SDC data are 2022 and 2023, the most recent available for a company that no longer reports.
5. Clinical Outcomes: Comfort Wins, Precision Still Lags
The evidence explains why adults like aligners and why orthodontists still keep brackets in the drawer. A 2026 scoping review of 119 studies found refinements were required in over 50% of aligner cases and 17.2% needed conversion to fixed appliances, with early-generation Invisalign studies reporting about 50% mean accuracy of planned tooth movements (Borzabadi-Farahani, Frontiers in Dental Medicine 2026). Accuracy varies by movement: rotations average about 65%, incisor torque 41.9% (maxillary) to 58.2% (mandibular), while arch expansion reaches 72.8% in the maxilla and 87.7% in the mandible, mostly by tipping. In mild cases aligners needed an additional 4.8 months versus fixed appliances.
Comfort and hygiene are where aligners score. A meta-analysis of 32 studies in BDJ Open found lower plaque (MD -0.437), gingival index (MD -0.233) and probing depth scores, less pain (SMD -0.419) and better quality of life at one week, one month and end of treatment (Kaur et al., BDJ Open 2026). A separate meta-analysis of 6 studies and 392 patients found about 0.5 mm less root resorption with aligners, though the certainty of evidence was rated low (Yildirim and Topbas, Healthcare 2026). For adults balancing treatment with work and appearance, that tradeoff (less discomfort, more refinements) is the whole value proposition.

Context note: the PLoS One review of adverse effects (PLoS One 2026) pooled 34 studies and 1,636 participants; its single-arm design measures aligner patients only, not a comparison with braces. The 50% accuracy figure is from early-generation studies within the review and newer materials, attachments and direct 3D printing (Align calls its palatal expander its first direct 3D printed orthodontic device) aim to raise it; see our 3D printing statistics.
6. Market Size, Forecasts and Spending Outlook
Market research firms cannot agree on the size of this market, and the spread is itself a finding. Precedence Research estimates the global clear aligner market at 11.12 billion dollars in 2026 rising to 113.64 billion dollars by 2035, a 29.59% CAGR (Precedence Research, Clear Aligners Market), while MarketsandMarkets projects only 11.04 billion dollars by 2031 at a 12.6% CAGR (MarketsandMarkets, Clear Aligners Market, September 2026). Precedence says adults were the dominant age segment and North America held 56.87% of the market in 2025.
The company guidance is far more sober. Align expects 2026 clear aligner volume up about 6%, ASPs flat to slightly down and total revenue up 3% to 4%, and its 2025 aligner ASP was 1,245 dollars per case. Consumer dental budgets are tight: US dental spending was 189 billion dollars in 2024, and the ADA Health Policy Institute reports consumer dental spending up just 1% year over year as of May 2026, with only 40% of working-age adults visiting a dentist in a year (ADA Health Policy Institute, Dental Care Market). Elective adult aligner treatment competes with other self-pay health spending, as seen in our GLP-1 weight loss statistics.

The ADA HPI quarterly survey (State of the US Dental Economy Q2 2026) covered 552 private practices and does not break out orthodontics.

- Data watch: Align does not publish an explicit adult share; the 64% figure is derived by subtracting 935.8 thousand teen and kid treatment starts from 2,611,280 case shipments, two measures that are close but not identical. Align’s Q2 2026 release gives no segment or regional volume numbers, only qualitative statements (double-digit growth in APAC, EMEA and Latin America, stable North America, double-digit DSO growth). Align’s own documents disagree on doctor counts: the 10-K reports about 130,015 active trained doctors (at least one case in 12 months) while the Q4 2025 release cites over 295 thousand active trained doctors; we use the defined 10-K figure. The AAO’s ‘one out of three’ adult share and 3.9 million children figure are undated on its press page. Market sizes conflict sharply (Precedence 11.12 billion dollars in 2026 at 29.59% CAGR vs MarketsandMarkets 11.04 billion dollars only by 2031 at 12.6%); Grand View Research and Fortune Business Insights pages were blocked or unavailable. SmileDirectClub data (2022-2023) and Byte data (2024-2025) are the most recent available for companies that exited. The Turkey survey had a 7.5% response rate. ADA HPI’s dental care market page cites consumer dental spending up 4% as of January 2026, while its Q2 2026 report cites up 1% as of May 2026; we use the newer figure. Straumann’s ClearCorrect volumes are not disclosed. Web search was unavailable during this research, so all sources were reached by direct fetch.
